Arizona Cycle Park Minor Releases & Temporary Guardian/Medical Consent Forms: If you are 17yr or younger attending a race or practice without a Parent or Guardian, these forms will be required. There are no exceptions. Please follow the instructions for each form. Bring them to sign-up.

2024 Annual Minor Release (Both Non-AMA & AMA): Required for riders 17yr or younger attending a practice or race at ACP without a Parent or Guardian. The form must be completed, signed by both parents and notarized.

Temporary Guardian / Medical Consent Form (Required for all events including AMA Events): Required for riders 17yr or younger attending a practice or race at ACP without a Parent or Guardian. We all would like to assume that nothing will happen, but you just never know. Having this information readily available will help expedite medical attention. Form must be one page. Page 1 must have Page 2 printed on the back. This form cannot be accepted stapled together.
